  • 3. TRN NO. HAP ID Print out copy or HAP ID (1 copy)
  • 4. All medical records of past major hospitalization or treatment
  • 5. Current medications, if any
  • 6. For Applicants treated for tuberculosis (TB) old X-Ray chest films with reports and full details of treatment.
  • 7. Up-to-date COVID vaccination certificate
  • 8. “E-mail ID” of your nearest relative (if you do NOT have one of your own)
Canada For Student, Visitor, Work Permit & Permanent Residency Required Document (s)
  • 1. Original Passport
  • 2. Photocopy of 1st & last pages of the Passport (2 Copies)
  • 3. UCI NO., Company Letters, College Offer Letter (Letter of Acceptance)
  • 4. All medical records of past major hospitalization or treatment
  • 5. Current medications, if any
  • 6. For Applicants treated for tuberculosis (TB) old X-Ray chest films with reports and full details of treatment.
  • 7. In case your passport is submitted at the VFS office or high commission, please bring an acknowledgement along with either of the following ORIGINAL national ID
    - Aadhar Card
    - Driving license
    - Up-to-date COVID vaccination certificate
United Kingdom (UK) Required Document (s)
  • 1. Original Passport
  • 2. Photocopy of First & Last Pages of the Passport (1 copy)
  • 3. Recent 2 Passport Size Photographs (United Kingdom (UK) Format) White Background (not more than 3 months old) The image of you from the crown of your head to your chin must be between 29mm and 34mm high.
  • 4. NO SUNGLASSES/NO CAP/NO HEADSCARF ALLOWED at the time of taking of the photograph.
  • 5. All details of past treatment for tuberculosis (TB) if any.
New Zealand Student, Work Permit & Visiting Visa > 1 Year
